個人資料收集聲明
此聲明為向您提供服務的資料用戶（喜樂童伴輔導及發展中心）告知您為何需要收集您的個人資料，打算如何使用，根據《香港個人資料（私隱）條例》的規定可以向誰提供資料，以及如何取得，查看和更正資料。 

《個人資料收集聲明》是本中心在向資料當事人收集個人資料之時或之前提供的聲明。資料當事人可能在知或不知情的情況下被收集資料。 

收集個人資料的目的
本會向您收集的個人資料，以用作向您提供所需的服務。 

自願提供個人資料
您提供個人資料與否純屬自願。當您向我們提供個人資料時，請確保這些資料是準確和完整的。如您未能提供準確和完整的個人資料，我們可能無法向您提供您需要的服務，也可能導致延期或被拒絕，如服務延誤或產生任何損失，需由閣下自行負責。 

個人資料之使用
申請人自願向本中心提供資料作為中心處理活動報名及日後通訊之用。申請人清楚知道，為確保活動之質素，整個活動過程會被攝錄，以作為本中心導師檢討及教學參考之用。申請人也同意本中心把過程中拍攝的照片用於網站及印刷品作為推廣宣傳之用。

披露個人資料
所收集的資料將保密處理。在未經您的同意前，我們不會出於任何目的向其他人士或機構披露所收集得關於您的個人資料。 

我們可能會在法律或其他政府和監管部門要求，或您明確同意披露時，披露您的個人資料。 

保存個人資料
為了滿足收集資料的目的，資料會被保存一段必要的時間。不再需要的數據將被銷毀。您有權查閱並要求永久銷毀本中心收集和儲存的任何個人資料。 

查閱及更正資料權利
您有權要求查閱及更正我們所持有關於您的個人資料。如您希望查閱或更正您的個人資料，請以書面形式提出。 

此個人資料收集聲明的更改
喜樂童伴輔導及發展中心保留隨時更新此個人資料收集聲明的權利，並在我們的網站上或以書面形式提供更新的個人資料收集聲明，以便您了解我們會收集的資料，我們會如何使用資料，以及在何種情況下資料會被披露。任何更改，更新或修改將在發佈後立即生效。 

若中英文版本有差異之處，以中文版本為準。

Personal Information Collection Statement
This statement provides you, as the data subject to whom the data user, PlayFellow Counselling and Development Centre (PlayFellow), provide services, with notice as to why your personal data is collected, how it is intended to be used, to whom the Data may be provided to, and how to access, review and correct the Data, in accordance with the requirements of the Hong Kong Personal Data (Privacy) Ordinance (“PDPO”). 

The Personal Information Collection Statement (PICS) is a statement given by PlayFellow to be presented to the individual, the data subject, on or before collecting the personal data from that data subject, whether known or unbeknown (e.g. through the use of cookies).  

Purpose of Collection
The information collected from you will be used by PlayFellow for all purposes related to your use of PlayFellow's services. 

Voluntary to Supply Personal Data
Provision of personal data by you is voluntary. When you provide personal data to us, please make sure that the data is accurate and complete. In the absence of your accurate and complete personal data, we may not be able to provide you with the services you require or it may also result in being deferred or rejected. If there is any delay in service or any resulting loss, it shall be the responsibility of the individual.

Use of Personal Data
Applicants voluntarily provide information to our center for the purpose of activity registration and future communication. Applicants are aware that in order to ensure the quality of the activities, the entire process may be recorded for review and educational reference by our center's instructors. Applicants also agree that the photos taken during the process may be used on the website and printed materials for promotional purposes.

Disclosure of Personal Data
The data collected will be kept in confidence. We will not provide your personal data to third parties for any purposes without your consent. 

We may disclose data when required by law or other government and regulatory authorities; or if explicit consent to such disclosure is given by the data subject. 

Retention of Personal Data
The Data will be retained for as long as necessary to fulfill the purpose for which the Data was collected. The Data which is no longer required will be destroyed. You have the right to access and request for permanent deletion of any personal data collected and stored by PlayFellow. 

Rights of Access and Correction of Personal Data
You have the right to request access to and correction of information held by us about you. If you wish to access or correct your personal data, the request for accessing personal data must be made in writing. 

Changes to this Personal Information Collection Statement
PlayFellow reserves the right to update this Personal Information Collection Statement at any time and will make available an updated Personal Information Collection Statement on our website or in writing so that you are always aware of what information we collect, how we use the information and under what circumstances the information is disclosed. Any such change, update or modification will be effective immediately upon posting. 

If there is any inconsistency or conflict between English and Chinese version of this statement, the Chinese version shall prevail.
